Perfil epidemiológico e laboratorial de mulheres até 25 anos com diagnóstico de lesão precursora do câncer de colo uterino atendidas em serviço de referência em Manaus.

Resumo

BACKGROUND: According to the International Agency for Research on Cancer for 2018, there were 570,000 new cases and 311,000 deaths from cervical cancer worldwide, with a percentage of 3.2% of all cancers. In Brazil, for the three-year period between 2020-2022, the number of new cases estimated is 16,590, showing a higher incidence in the northern region, with 21.20 / 100,000 women, with 33.08 / 100,000 being the estimated rate for Amazonas. OBJECTIVES General: The screening of this neoplasm is necessary to diagnose a precursor lesion, in a timely manner and to define an ideal conduct. OBJECTIVES Specifics: The diagnosis of these injuries requires organized strategies to segment the target population, providing adequate treatment to prevent new cases of invasive diseases. Considering new adaptations and revisions of the current regional protocols for cervical cancer, may be a strategy for screening women below 25 years of age, considering a population with persistent human Papillomavirus lesions, in need of consultation and regular screening. METHOD: This work, developed at the Araújo Lima outpatient clinic in Manaus, is a cross-sectional retrospective, with consultation of medical records. Epidemiological data, cytological, colposcopic and histopathological exams of patients seen at the lower genital tract pathology service were analyzed in Manaus between 2014 and 2018. A database was created in the Excel program to insert socio-demographic and gynecological variables, in addition to the results of cytological, colposcopic and histopathological exams that were also tabulated. RESULTS: It was observed that 60.5% of women, with high-grade intraepithelial lesion in cytology, had confirmation in histopathology. Excisional treatment occurred in 45.9% of patients. A total of 97.4% of excised parts had injury-free margins. CONCLUSIONS: The research data propose a reflection on the ideal coverage of screening in Amazonas carried out in women below the age of screening, with risk factors that can interfere in the evolution of the injury. The national consensus proposes a population-based screening, however, the results are a cause for concern when considering the particularities of the northern region, where young women with precursor lesions could have evolved without an appropriate and timely approach, others would not even be able to reach a health center from the interior of the state.
