The prescription of medications in intensive care units (ICUs) requires special
attention, especially in patients undergoing neurosurgery, due to the high risk of drug interactions. This study aimed to evaluate medication prescriptions used in
neurosurgical patients admitted to the adult ICU of a university hospital, identifying the frequency and severity of potential drug-drug interactions (pDDIs). This is a
retrospective study analyzing 246 medical prescriptions issued between January and December 2024. Data were collected from the AGHU system and analyzed using the UpToDate database. Drug classification followed the WHO's ATC system. A total of 2,742 medications were identified, with a predominance of nervous system drugs (28.5%). Most interactions were classified as risk C (43%), followed by risk D (25%) and risk X (7%). Medications such as quetiapine, methadone, propofol, and midazolam were involved in the most severe interactions. The results highlight a scenario of polypharmacy and therapeutic complexity in neurosurgical ICUs, emphasizing the importance of continuous pharmacotherapeutic monitoring and the clinical pharmacist’s role in preventing adverse events and promoting the rational use of medicines.
