Avaliação da qualidade de vida relacionada à saúde geral e bucal, e seus fatores associados em pessoas trans no Amazonas

Resumo

Transgender people, when compared to the cisgender population, experience several situations of discrimination that negatively impact their daily lives. Psychosocial stress has the potential to affect the physical, psychological and wellbeing domains, which contribute to the impairment of HRQoL. Furthermore, stress and hormonal changes peculiar to trans people can impact their oral health, including the periodontium. Despite the increase in scientific interest in trans experiences and HRQoL, studies investigating HRQoL in this population are scarce. This crosssectional observational study evaluated HRQoL, its associated factors and how these outcomes relate to periodontal health in trans people in a reference center in the city of Manaus, Amazonas. Data were collected through questionnaires applied through interviews, which included questions and validated instruments for measuring the variables. The model included: Gender, Chronic conditions; Health-related behaviors; HRQoL, OHRQoL, Socioeconomic characteristics; Social support and self-reported periodontal status. 71 participants were included, with 50.70% identifying as trans women, 40.85% trans men, 2.8% transvestites and 5.6% non-binary, with an average age of 30.08 years. The physical component of the SF-12 presented a score of 44.56 (SD=7.82) and the emotional component, 38.41 (SD=11.08). The emotional aspect was the most affected domain. Transgender people who reported having paid work had better HRQoL (mental component), while having lived on the streets was associated with worse physical HRQoL. The analysis showed that worse selfreported periodontal condition was associated with worse scores on the OHIP-14 and the mental component of the SF-12. It was concluded that trans people using a specialized health service, located in the city of Manaus, Amazonas, Brazil, had worse HRQoL than the Brazilian population, with the mental component being worse than the physical component. The HRQoL of trans men was similar to that of trans women and transvestites. Transgender people who declared their race/color to be white had better OHRQoL. Having a worse self-reported periodontal condition was associated with worse OHRQoL and the worse mental component of HRQOL.
