Introduction: Kinesiophobia, the disabling fear of movement, is frequently associated with chronic low back pain, impacting functional recovery. Objectives: This study evaluated whether exercise practice influences the level of kinesiophobia and postural instability in patients with chronic low back pain. Methodology: Fifty-six volunteers aged between 18 and 70 years were recruited and assessed using the SBST questionnaire to classify the risk of poor prognosis. Kinesiophobia was measured by the Tampa Scale, while postural instability was assessed using the Wii platform, analyzing the body's center of pressure. The intervention protocol included eight exercise sessions focused on muscle activation of the abdominal wall, with progression of functional exercises over four weeks. Results: Of the 56 recruited, 21 completed the protocol. The mean kinesiophobia score decreased slightly from 46.87 to 45.53 points, without statistical significance (p=0.54). Likewise, postural instability increased from 35.72 mm² to 36.37 mm², also without statistically significant difference (p=0.81). Difficulties in using the evaluation platform and lack of regular attendance by volunteers compromised the analysis of the results. Conclusion: It is concluded that, although the literature recommends exercises for chronic low back pain, the number of sessions and intervention time in this study were insufficient to cause significant changes in the variables evaluated, especially kinesiophobia and postural instability.
