Protocolo de manejo pré e pós-operatório em cirurgia bariátrica do Hospital Universitário Getúlio Vargas – HUGV- EBSERH

Abstract

BACKGROUND: Obesity is a chronic, progressive disease, with global incidence, difficult to manage and represents a serious threat to individual health as a whole. Due to these characteristics, several approaches have been applied to take care of these patients, with unanimity about the need of a multidisciplinary, customized and continuous assistance. OVERALL AIM: The Bariatric Surgery Department at Getúlio Vargas University Hospital (GVUH) – EBSERH from Federal University of Amazonas has achieved good results and guaranteed the best for patient outcome, but the lack of systematization in some medical conducts remains a challenge. SPECIFIC AIM: To develop a protocol for pre-, intra- and postoperative routines for patients undergoing bariatric surgery at GVUH. METHOD: A review of literature was conducted and a protocol was applied to specialists using the Delphi method, which is described as a decision-making method for complex issues, without the need for face-to-face meetings. As an advantage, expert opinions are collected individually and anonymously, without researcher or other physician interference, in order to include different views and experiences. This allows adapting their responses to the department reality and to the profile of patients who are assisted here. RESULTS: Thirty-one experts answered the questionnaire, aiming to obtain an agreement rate greater than 65% on the proposed questions. Through the results obtained from the form and with the review of literature, the protocol for pre- and postoperative management of bariatric surgery was prepared, and will serve as a guide for the medical conducts of the Bariatric Surgery Department. CONCLUSIONS: Gathering of expert opinion through the Delphi method allowed to know other realities, especially the medical decisions adopted by professionals working in the bariatric surgery field across the country. It brought new information that, in association with a review of the literature, allowed the elaboration of a customized protocol for the management of our local bariatric surgery patients, from admission to postoperative follow-up period, always with updated data from the literature associated with expertise from specialists.
