This study reports the clinical case of an endodontic treatment of element 36 with the morphological anomaly type “C”, diagnosed with pulpal necrosis. After anesthesia and absolute isolation, surgical access was performed with spherical and conical drills followed by catheterization with #10 and #15 K-files, with a provisional working length of 18 mm. The odontometry was obtained by means of an electronic foraminal locator, the measurements collected were 21 mm in the mesiolingual, mesiobuccal and distal canals. The TruNatomy system was used to instrument the conduits and irrigation was performed with 2.5% sodium hypochlorite. Patency was performed in all conduits with a #15 k-file and final irrigation with 17% EDTA followed by sodium hypochlorite. The conduits were dried with absorbent paper and filled with gutta percha cones corresponding to the system used and the filling was performed using the thermocompaction technique. Cleaning of the pulp chamber, provisional restoration and final X-ray for proservation were performed. The patient was followed up for 6 months and the element remains asymptomatic and without periapical alterations.
