Perfil epidemiológico dos atendimentos com indicação cirúrgica por fraturas acidentais, de um serviço de ortopedia e traumatologia em urgência e emergência da cidade de Manaus - Amazonas

Resumo

BACKGROUND: The availability of external fixators in the hospital base of the study is sometimes insufficient in relation to the demand for emergency care, which results in inappropriate use of the material offered. Therefore, identifying the causes of this situation may help in the resolution of cases, in improving the prognosis, and in improving the distribution of resources in relation to the acquisition of material under statistical bases. OBJECTIVES: To determine the occurrence and frequency of accidental fractures treated at the Emergency and Urgency Unit at the Orthopaedics and Traumatology Service of the Hospital and Emergency Room Dr. Aristoteles Platão Bezerra de Araújo. As well as, to develop an epidemiological basis for fractures due to accidents by affected segment, and to list fractures requiring immediate external fixation, the type of fixator used in the injured segment in order to demonstrate the need to supply different types of fasteners. METHOD: Analysis of a total of 1,208 records and records and a list of material provided with records of fracture occurrences from January 1 to December 31, 2016. It consisted of a descriptive, cross-sectional retrospective study. The data was tabulated initially using Excel spreadsheet and then building database using IBM SPSS Statistics version 21.0 and Minitab. The main variables of interest were evaluated, and the descriptive and exploratory analysis was performed through frequency for categorical variables, mean and standard deviation for numerical variables. RESULTS: Of the records collected, 64.9% (n = 784) were males. It was observed that 80.7% (n = 113) of the group of subjects submitted to orthopaedic emergency and emergency procedures were between 20 and 49 years old. The majority, 78.5% (n = 102) of the operated ones, were in the lower limb; of these 65.5% (n = 81) were in the tibia. In 92.9% (n = 130) they placed external fixators and in 16% (n = 12) the used fasteners were inappropriate for the fixed segment. CONCLUSIONS: The results suggest that the number of (140) adults who underwent emergency surgical procedures in the study unit, using external fixators, is relevant, totaling 12% of individuals treated for fractures. In 16% of cases, inappropriate use was found, especially in the upper limbs. There is a need to increase inventory control, optimization of collection, analysis and use of occurrence records. In addition, urgent installation of a continuous information system that facilitates the decision making of managers, regarding the availability and variety of external fasteners and ancillary components for emergency urgency.
