Avaliação da capacidade institucional para atenção às condições crônicas nos serviços de saúde especializados em idosos na cidade de Manaus-AM

Abstract

Introduction: Changes observed in the profile of SUS users have significant impact on the population's health, bringing new forms of health care, mainly due to the increased burden of chronic noncommunicable diseases. To identify these transformations, emerge the necessity for continued assessment of health processes and applied policies. However, there is a mismatch between the increase in chronic conditions and the work and management process, which still privilege acute conditions or the sharpening of chronic conditions. Therefore, the objective of the study was to evaluate the capacity of attention to hypertensive patients in health services specialized in the care of the elderly population based on the Care Model for Chronic Conditions allowing the identification of potentialities and weaknesses in the care administered to these users. A cross-sectional, descriptive and quantitative study was performed. The instrument used was the Assessment of Chronic Illness Care, applied between April and July 2019, designed to assist in the planning of strategies aimed at improving the care of chronic diseases and to evaluate the level of implementation of these strategies in the system, allowing the stratified increasinglyclassification: limited support, basic, reasonably or fully developed chronic illness care. Were applied 53 questionnaires to health professionals from institutions specialized in elderly care in the city of Manaus. After analysis it was observed that the institutions CAIMI Dr. Paulo Lima, CAIMI Dr. André Araújo and CAIMI Ada Rodrigues Viana reached basic support for elderly hypertensive and the Polyclinic UnATI / UEA showed a reasonably capacity. The main institutional weaknesses identified were related to the dimensions: (a) community linkages; (b) decision support; (c) clinical information system and (d) integration of Chronic Care Modelcomponents. Related potentialities are: (a) vast experience of health professionals in care for the elderly; (b) team training; (c) self-management support and (d) implementations in service delivery design. Conclusion: The results showed a basic capacity to care for hypertensive users in specialized services to the elderly population in the city of Manaus.
